Liverpool Parent and Carer Forum (LivPaC) are wanting to gather parent carer opinions about services relating to children and young people aged 0-25 with an additional need or disability. This can be anything across Education, Health or Social Care and we want to hear the good the bad and the ugly. Please use this form to detail your experiences, giving as much detail as you can. The feedback that you share will be used to drive forward our Topics of Importance which are delivered to Liverpool City Council and Liverpool CCG regularly, and will inform the strategic boards and working groups that we sit on, shaping future service provision to meet local need.
